Federal Inquiry Initiated into Avride’s Autonomous Vehicle Safety Concerns
The National Highway Traffic Safety Governance (NHTSA) has commenced a complete inquiry into avride, an autonomous robotaxi service linked to Uber, after uncovering more then a dozen collisions and one incident involving minor injuries.
Examination of Collision incidents and System Limitations
The Office of Defects Investigation (ODI) within NHTSA identified 16 separate accidents connected to flaws in Avride’s self-driving technology. The system exhibited challenges with executing lane changes safely, responding correctly to vehicles occupying the same lane, and detecting stationary obstacles on the road.
In every case, a safety operator was present in the driver’s seat; though, Avride has yet to explain why these monitors failed to prevent or lessen the severity of these crashes. The company maintains that it adhered strictly to regulatory mandates by reporting all incidents under NHTSA’s 2021 Standing General Order for automated driving systems.
Company Measures and Performance Improvements
Avride disclosed that it introduced targeted technical corrections alongside operational adjustments addressing each reported event between December 2025 and March 2026. these interventions reportedly improved system reliability. Despite an increase in total miles driven during this timeframe, the frequency of incidents per mile has steadily decreased.
Robotaxi Expansion Across Texas: A Closer Look at Incident Locations
This investigation coincides with Uber’s rollout of robotaxi services utilizing Avride vehicles in Dallas-a city where numerous collisions have been documented-as well as Austin. At least one crash involved passengers inside an autonomous vehicle at the time of impact.
A notable injury occurred in December 2025 when an Avride-operated Hyundai Ioniq 5 grazed open doors on a parked pickup truck in Dallas; one occupant sustained minor injuries but did not require hospital care. Another accident from that month involved an attempted lane change around a parked truck which resulted in contact with a neighboring van causing damage to both vehicles.
Patterns Emerging from Incident Data

This federal probe arises amid heightened national attention as multiple companies accelerate deployment and testing of self-driving technologies across urban areas throughout the United States. As an example, Waymo is currently under scrutiny by both NHTSA and the National Transportation Safety Board following allegations related to school bus interactions and also involvement in a January collision injuring a child near Santa Monica Elementary School.
“Video evidence reviewed by ODI reveals repeated instances where autonomous vehicles abruptly shifted lanes directly into adjacent traffic or failed to slow down for slow-moving or stopped cars ahead,” officials stated after initial analysis.
